The electronic frailty index has been a hugely successful tool, helping GPs across the UK to care for frail patients. Recently the eFI+ was released through EMIS and TPP, giving predictions of four key patient outcomes to help guide treatment plans. In particular, the model to predict serious falls has attracted attention from ICBs across the UK and is being used for population health management to deliver preventative care. We will talk about how the unique data within Connected Bradford was key to the successful delivery of this project, and give everyone a sneak-peek of other projects that we’re just starting now that arose from the eFI+ work.
